Former Adirondack Tri-County center joins Elderwood community

NORTH CREEK, NY – Elderwood at North Creek, formerly Adirondack Tri-County Nursing & Rehabilitation Center, becomes the newest member of the Elderwood care community, effective Monday, January 22, 2018. The skilled nursing facility is located at 112 Ski Bowl Road, North Creek, NY.

Elderwood parent company Post Acute Partners, under the leadership of Co-CEOs Dr. Jeffrey Rubin and Mr. Warren Cole, is pleased to become a community health care partner with its second facility in the Adirondack area. The property will maintain capacity at 82 beds.
